Job Summary We are looking for someone to work in the Westhaven Apartments in N.
Baltimore, Ohio who can maintain confidentiality and provide general assistance and advocacy related to supportive and social services to residents.
The ideal candidate will have experience working with the elderly and or persons with disabilities.
Essential Job Functions Assist primarily individuals sixty (60) and older as well as persons with physical disabilities to find and maintain safe, affordable accessible housing to meet their needs.
Conduct annual assessments to assess the needs of residents.
Utilize assessment data to provide appropriate interventions to improve residential outcomes and connect residents with programs and engagement activities that address their most pressing needs.
Monitor priority residents quarterly.
Conduct safety checks/calls to confirm residents are functioning safely in the home environment.
Act as a liaison between residents, management & social agencies.
Assist residents in building support networks and provide information on concerns.
Maintain residents' records and complete reports as needed.
Work with supervisor to develop programming or activities that may not be offered such as (e.
g.
, flu shot clinic, vitals clinic, podiatry clinic, exercise class, nutrition class, mobile vision clinic, craft class, holiday parties, book clubs, etc.
) Work with residents to promote a positive, non-judgmental environment that encourages cooperation, engagement, independence, resident conflict resolution, and volunteerism.
Qualifications:
Degree in gerontology, disability studies, social work, nursing, public administration, healthcare administration, geography, and planning, and/or related field preferred.
Minimum of one year of housing, information, referral, gerontology, independent living, or related experience.
Excellent customer service skills.
Basic computer and mathematical skills.
Strong oral and written communication skills.
Reliable transportation, insurance, valid driver's license, and successfully pass criminal background check and drug/alcohol screening.
